Cardiology Nurse Practitioner
Hours: M-F, 9 AM - 5:30 PM Rotating Weekends
OVERVIEW:
Provide NP level care to patients in a clinic setting.
SUMMARY:
Seeing 20 plus patients per day
Create a rapport with children, parents, and adult patients by providing an exemplary healthcare experience
Work with Family Medicine Physician, MAs, and Office Manager to uphold the goals and mission of the practice
DUTIES & RESPONSIBILITIES:
Provide comprehensive primary and multi-specialty care to pediatric and adult patients in an outpatient clinic setting.
Conduct patient histories, physical examinations, assessments, and follow-up visits.
Diagnose and manage acute and chronic medical conditions.
Evaluate and manage patients with cardiovascular conditions and cardiac risk factors.
Review and interpret relevant laboratory results, diagnostic testing, and patient medical history.
Develop and implement individualized treatment and care plans.
Monitor patients with chronic conditions and provide appropriate follow-up and education.
Recognize changes in patient condition and determine when additional evaluation, testing, or specialist referral is appropriate.
Provide patient and family education regarding medications, treatment plans, lifestyle modifications, and preventive care.
Establish a strong rapport with children, parents, and adult patients while providing a positive healthcare experience.
Collaborate with Family Medicine Physicians, Medical Assistants, and the Office Manager to coordinate patient care.
Maintain accurate and timely clinical documentation and medical records.
Participate in preventive care, health maintenance, and chronic disease management.
Provide coverage at the Brooklyn, Staten Island, and Manhattan locations as needed. Support the practice's commitment to high-quality, patient-centered care and long-term patient relationships.
